- Two color photo 35 x 45mm 80% face white background in matt finish, pls write applicant name & ppt no behind on photo with marker pen..
- Visa application form signed by blue ball pen only.
- Confirmed Ticket copy.
- Personal bank statement copy for last 03 month with balance of above Rs. 50,000/-
- Visiting Card copy of applicant as employment proof.
- Vaccination certificate copies both doses.
- Hotel Name, address with postal code & contact number
- Visa form & V39 signed by Pax only with blue pen as per Passport
